Please complete this form, print it, then mail it to the Women's Business Center at Cornerstone Alliance's mailing address (PO Box 428, Benton Harbor, Michigan 49023). This form helps the WBC staff to get to know you and the type of business assistance you need. The information contained within this form is entirely confidential.

A one-on-one counseling session will be scheduled to learn more about your business venture and to provide additional information on the WBC's services.
